Medical Records Clerk

Medical Records Clerk works assisting the providers and clinic supporting staff to ensure the EMR documentation that is entered accurately and kept up to date, while protecting confidentiality and security of patient health information.

Job Responsibilities/Duties

  • Pulls records from suites for scanning into EMR System.
  • Following established guidelines, scans documents with accuracy into the EMR system.
  • Participates in determining appropriate name convention of tabs associated with scanning.
  • Sorts incoming chart/documents assuring that all are attached to the correct patient.
  • Retrieves medical records on request, carefully identifying record and following release of information guidelines.
  • Performs duties in a professional manner while exhibiting a courteous and cooperative demeanor with patients, co-workers, management, and the public.
  • Provides suggestions regarding process or quality improvement to department manager.
  • Answers phone calls, takes messages, duplicates documents, and assists with other clerical duties as requested.
  • Maintains strict confidentiality both internally and externally when medical information is requested. 
  • May be called upon to assist with hardware and/or software setup as it pertains to the EMR department.
  • Attends meetings as required.
  • Other duties as assigned.


Minimum Education: High School Diploma.

Minimum Experience: One to two years (1-2) years of health care delivery system experience. Knowledge of health data structure, information resource management, and quality evaluation and management preferred
