Community Liaison

We are hospitals and affiliated medical groups, working closely together for the benefit of every person who comes to us for care. We build comprehensive networks of quality healthcare services that are designed to offer our patients highly coordinated, personalized care and help them live healthier lives. Through collaboration, we strive to provide all of our patients and medical group members with the quality, affordable healthcare they need and deserve.

Under the supervision of the Vice President of Business Development, the Community Liaison needs to be a detail-oriented person. Duties will include aligning nursing homes with hospitals and physicians to collaborate services needed for both hospital and nursing homes. Establish long term relationships with Administrators and Nursing Home Coordinators to assist patients receiving urgent hospital care.



Job Responsibilities/Duties

  • Works with internal and external audiences to build awareness and preference for hospital and its services,​ while improving the health of the community.
  • Leads the planning of special events geared to improve health and promote hospital services.
  • Prepares support materials as necessary to support outreach efforts.
  • Documents results of events to demonstrate effectiveness at changing health behaviors, enhancing​ consumer image of hospital or increasing utilization of hospitals services.


Qualifications

Minimum Education: Minimum of a High School Diploma or equivalent. Bachelors Degree preferred; College courses in business administration and/or healthcare administration preferred.

Minimum Experience: Nursing home care experience or experience in a multi-disciplinary clinic setting preferred, one year specialty area experience preferred.
