Clinical Pharmacist

The  Clinical Pharmacist provides disease management support for patients enrolled in Prospect Medical Group’s Diabetes Management Program, attends interdepartmental clinical rounds to provide clinical feedback on medication use and resolve issues, develops and implements pharmacy utilization management (UM) strategies, and provides physician and member education for various pharmacy programs.

Job Responsibilities/Duties

  • Provide disease management for patients enrolled in the Diabetes Management Program or medication reviews for high risk diabetic patients.  Refer patients to Health Education and specialists as appropriate.  Review patient profiles to ensure that STAR measures are met by providing solutions for missing labs, health screenings, or medications.
  • Attend biweekly High Intensity meetings and/or daily interdisciplinary clinical rounds to provide pharmacy recommendations. Counsel patients regarding appropriate uses of medications and resolve medication issues.  Contact physicians for necessary therapeutic interventions. Document communications with physician offices and members regarding MTM recommendations in appropriate pharmacy & case management systems.
  • Provide support for the Pharmacy & Therapeutics Committee, Utilization Management Committee, and Quality Management Committee. Develop utilization management protocols for drugs including injectables, infusion therapies, chemotherapeutic agents, and vaccines.  Present new drug reviews, therapeutic class reviews, drug usage guidelines, and clinical programs to the Pharmacy & Therapeutics Committee.
  • Provide physician & member education to promote generic prescribing.  Make clinical interventions to promote part D pharmacy measures. Educate members on medication adherence by identifying root causes for non-adherence issues and providing tips and tools to foster medication compliance. 


Minimum Education:  Graduate of an accredited School of Pharmacy recognized by the California State Board of Pharmacy. An advanced degree such as a Doctor of Pharmacy or Masters of Sciences is preferred.

Minimum Experience: A candidate with ambulatory care experience or MTM is preferred. A prior pharmacy residency program or managed care experience (e.g., health plans, pharmacy benefit management companies, medical groups) is preferred.

Req. Certification/Licensure: The candidate must hold an active pharmacist license in the state of California.
