Post Discharge Coordinator

We are hospitals and affiliated medical groups, working closely together for the benefit of every person who comes to us for care. Daily telephonic Post-Discharge calls to all members within 24 - 48 hours after discharge from Acute Setting and Skilled Nursing Facilities. Ensure member receives an outpatient appointment and conducts follow-up with patient or caregiver.

Job Responsibilities/Duties

  • Calls to members are completed within 24-48 hours after discharge.
  • Complete post discharge questionnaire on all discharge member and maintain accurate documentation.
  • Ensure All Senior members are scheduled post-acute follow-up in the Care Plus clinic within 24-48 hours post discharged.
  • Ensure appointments with PCP and or indicated SCP is scheduled no later than five (5) days post discharge or within the timeframe defined by the discharging physician.
  • Warm Transfer to a case manager regarding clinical information to ensure communications.
  • Confirm with member if discharge orders such as ancillary providers, DME, etc. have been delivered.
  • Returns member calls within 24 hours from receipt to ensure member satisfaction.


Minimum Education: High School graduate required.

Minimum Experience: Medical Assistance and Medical Terminology experience preferred.

Req. Certification/Licensure: None.
