Pharmacy Coordinator - DMP

The main responsibilities of the Pharmacy Coordinator includes: processing referrals for program enrollment; providing administrative support for all disease management programs including regular reporting; assisting pharmacists in completing medication review for patients with complex therapies; conducting initial disease management assessments for new enrolled members.



Job Responsibilities/Duties

  • Processes internal and external referrals for disease management programs for anticoagulation, CHF, COPD, and diabetes by contacting PCPs to obtain a physician collaborative agreement, clinical notes, and medication profiles. 
  • Administers initial disease management assessments with newly enrolled patients.
  • Schedules patient appointments for initial & follow-up consultations with a pharmacist and health educators. 
  • Contacts physician offices to ensure drug recommendations are resolved.
  • Prepares a packet with pertinent clinical information for the pharmacist’s consultation for all other disease management referrals (e.g., COPD, CHF).
  • Arranges compliance packing for medications through Prospect’s designated pharmacy network with a free delivery set up as appropriate. 
  • Handles paperwork required to transfer medications between pharmacies. 
  • Works in collaboration with Prospect’s pharmacy referral coordinators to process diabetic supplies through contracted vendors. 
  • Provides administrative support for Case Management, Disease Management and Nurse Practitioners for collaboration on all disease management programs.
  • Maintains the internal tracker for the completion of program enrollment, pharmacist consultations, or clinical /quality gaps for reporting purposes.
  • Provides regular reporting on patients who are not at target goals based on CMS STAR metrics.


Qualifications

Minimum Education: High School Diploma or General Education Development (GED) required.

Minimum Experience: Prior work experience in pharmacy setting (i.e. retail pharmacy) or managed care setting is required. Bilingual in Spanish preferred. WPM -50 (minimum). Working knowledge of MS Word and Excel

Req. Certification/Licensure: CA pharmacy technician license.
