Nurse Practitioner - DMP

The Disease Management Nurse Practitioner will provide high quality, evidenced based patient care to complex and high-risk patients with the primary goal and objective of providing best-in-class patient care, reducing avoidable hospitalizations, and decreasing unnecessary emergency department utilization. As a member of the care team, the Nurse Practitioner (NP), in collaboration with a interdisciplinary team, will be responsible for delivering effective clinical services for members with chronic conditions like Heart Failure, Chronic Obstructive Pulmonary Disease, Diabetes, etc. Compliance with all HIPAA regulations and maintain security of protected health information (PHI) is required.
Job Responsibilities/Duties
Qualifications
Minimum Education: Graduate of an accredited master’s degree in nursing (MSN) program. Completion of Nurse Practitioner (NP) accredited program.
Minimum Experience: Two (2) plus years of post-graduate experience working as a Nurse Practitioner in an Adult, Family, Internal Medicine, Geriatric, Palliative or Hospice setting. One (1) plus years of nursing experience in Mental Health, Behavioral Health, Psychiatry or similar Advanced knowledge of and experience with symptom management. Knowledge of clinical standards of care related to their field. Experience dealing with a complex set of patients with a relatively high level of acuity. Strong leadership skills and ability to function in an interdisciplinary team are essential. Access to reliable transportation that will facilitate travel to client and/or patient sites within a designated area. A high degree of organizational skills, ability to set priorities, manage multiple demands and the ability to complete tasks under strict timelines is required. Ability to establish therapeutic relationships with patients and families. Basic computer literacy to operate a computerized client/family information system, word processing and email. Effective written and verbal communication skills. Demonstrated ability to be self-directed, and motivated. Must be willing to provide in-home patient care and travel within assigned territory. Must be willing to be on-call, minimally 1 week per month.
Req. Certification/Licensure: Current, unrestricted RN/NP State License and State Driver’s License. DEA License and furnishing privileges with the State Board. Board Certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC). Current AHA Basic Life Support.
