Inpatient Coordinator

The Inpatient Coordinator supports the inpatient case manager to create a seamless transition for hospitalized patients and facilitate customer satisfaction. Documents in electronic medical record systems as directed.



Job Responsibilities/Duties

  • Reviews daily hospital census and enter any new admissions into electronic referral system and notifies inpatient case manager of admission.
  • Enters all referral requests and approve referrals that are on the “Approval” list for coordinators.
  • Documents all pertinent information in required electronic databases.
  • Uploads all Hospital Documents post discharge into the appropriate documentation system.
  • Notifies patient’s assigned PCP of hospital admission and discharge.
  • Manages the inpatient call queue.
  • Manages work voice mail and responds to messages within 24 hours.
  • Notifies the discharge coordinator of discharges.
  • Assist with setting up temporary contracts for new providers by obtaining all necessary information and forwarding to the contracting department by email or via the ‘pending contracting” queue in IDX.
  • Facilitate referrals for patient education to appropriate internal resources (health education, disease management programs, etc.)
  • Special duties:  Assist with special projects as assigned by management.


Qualifications

Minimum Education: High School Diploma.

Minimum Experience: HMO/Medical Group experience preferred.

Req. Certification/Licensure: Certified Medical Assistant preferred. Medical Terminology certificate preferred.
