Disease Management Program (DMP) Educator

This position has accountability for the direct provision of member education, skill development, and ongoing support for members identified to participate in PMS’s disease management programs (DMPs).  The DMP educator completes all relevant DMP educator assessments in the case management platform. The DMP educator participates in interdisciplinary care team meetings as appropriate and provides progress to DMP goal information on an ongoing basis to the care team.  The DMP educator interfaces with the member’s case manager and providers as required by protocols and appropriate to care planning and care outcomes.  Feedback is provided to the clinical development team on an ongoing basis and as requested to facilitate program improvement.



Job Responsibilities/Duties

  • The DMP educator outreaches member, conducts the appropriate DMP assessment(s), and documents according to approved protocols in the care management platform.  DMP assessments include but are not limited to the following: disease condition or need; medication adherence; quality of life; and depression and other behavioral health screening assessments.  The DMP educator develops a DMP action plan consisting of member-negotiated goals that support the DMP assessments and the Interdisciplinary Care Plan for the member.
  • The DMP educator schedules and implements telephonically self-management education, skills development, and problem-solving enhancement for members with specified chronic conditions (Heart Failure; COPD; CKD; and Diabetes).  Once a member attains DMP goals, the DMP educator provides ongoing support to facilitate motivation and encouragement to sustain new behaviors.  The DMP educator employs best-practice education and counseling approaches with members (Motivational Interviewing; Health Belief Model; Stages of Change; Social Learning Theory; etc.)
  • The DMP educator participates in the Interdisciplinary Care Team meetings as appropriate and shares pertinent DMP member information with care team as needed. The DMP educator messages the member’s assigned case manager when the DMP goals have been developed and/or achieved. The DMP educator facilitates the transition of members with severe symptoms and/or perceived de-compensation to a clinician for further evaluation and potential intervention. During all work with the member, the DMP educator facilitates “warm transfers” to case management and/or clinician as appropriate. The DMP educator facilitates care coordination to ensure members have the medical care and resources needed for chronic condition(s).
  • The DMP educator provides ongoing feedback to the development team on program assessments, goals, interventions, evaluation procedures, and workflow processes to facilitate continued program improvement.  The DMP educator provides feedback on all program member and provider components and may participate in development as requested.


Qualifications

Minimum Education: Bachelor's degree in Nursing, Health Science or other related field desired. ​If non-clinical, Master's degree in Health Education, Public Health, Social Work or other related field is required.

Minimum Experience: One (1) year of disease management or health coaching experience, preferably in a managed care environment. Knowledge of self-management skills associated with chronic illness clinical practice guidelines (Heart Failure, COPD, CKD, Fall Prevention and Diabetes as examples) and the theories of change associated with the attainment of self-management goals. Skilled in motivation interviewing and patient engagement strategies. Bilingual in English/Spanish is highly desired. Demonstrated ability to meet performance expectations under managed care contracts.

Req. Certification/Licensure: CCP, CDE, AE-C, CHES and/or MCHES preferred.
