Disease Management Program (DMP) Coordinator

The Disease Management Program Coordinator is responsible for providing program support for team members and patients enrolled in Disease Management programs.   Tasks include but are not limited to the following: DMP enrollment and processing of new enrollees, maintain/track Protected Health Information (PHI) of identified high risk members; conduct outreach, engagement and education of contracted providers and members; use of and data input to medical management electronic systems; and manage provider and member program materials.  

Job Responsibilities/Duties

  • The DMP Coordinator will process incoming referrals from various referral sources and determine eligibility for the program.  DMP Coordinator will conducts member outreach to invite and encourage DMP participation and document outreach and other notes in Electronic Medical Record (EMR) and other databases.  He/she will gather all pertinent clinical historical data and/or medical records and upload into appropriate system.  
  • The DMP Coordinator mails relevant DMP materials to members and documents in organization’s care management platform in accordance with program protocols. Will manage DMP supplies and resources to ensure adequate supplies of provider education tools and member education materials.  
  • The DMP Coordinator will maintain and update the master program membership file and other program metrics tracker for each disease management program (DMP). 
  • The DMP Coordinator manages the Tele-monitoring component of DMP for program participants that agree to participate. Coordinator enrolls members upon receiving signed agreement, follows up to ensure receipt of Tele-monitoring equipment, monitors all Tele-monitoring readings and alerts Case Manager as needed, and facilitates discharge from Tele-monitoring as directed by DMP Educator or Case Manager.
  • The DMP Coordinator administers DMP post program evaluation questionnaires.
  • The DMP Coordinator will perform other tasks as assigned by DMP Manager.


Minimum Education: High school graduate required; Associate in Arts degree preferred.

Minimum Experience: Experience in healthcare preferred. Experience in effective outreach and engagement. Excellent communication and interpersonal skills. Bilingual in English/Spanish is a plus. Experience working in electronic data systems and MS Excel. Demonstrated ability to meet productivity standards within a highly demanding work environment. Ability to work flex hours as needed to meet outreach productivity standards and member needs.
