The Disease Management Program Coordinator is responsible for providing program support for team members and patients enrolled in Disease Management programs. Tasks include but are not limited to the following: DMP enrollment and processing of new enrollees, maintain/track Protected Health Information (PHI) of identified high risk members; conduct outreach, engagement and education of contracted providers and members; use of and data input to medical management electronic systems; and manage provider and member program materials.
Minimum Education: High school graduate required; Associate in Arts degree preferred.
Minimum Experience: Experience in healthcare preferred. Experience in effective outreach and engagement. Excellent communication and interpersonal skills. Bilingual in English/Spanish is a plus. Experience working in electronic data systems and MS Excel. Demonstrated ability to meet productivity standards within a highly demanding work environment. Ability to work flex hours as needed to meet outreach productivity standards and member needs.
