Disease Management Program Case Manager (DMP CM)

The Disease Management Program Case Manager (DMP CM) is accountable for providing clinical care management and coordination support to high-risk members with chronic conditions. This position is responsible for members enrolled in disease management programs in order to reduce preventable healthcare utilization in the assigned population.  Responsibilities include providing direct patient assessments, care management, coordination of services, and triaging to appropriate clinical and non-clinical resources available to members, including social and behavioral health resources. The DMP CM will support the management of members identified across the continuum of care, including newly diagnosed individuals and those with recent hospitalizations, to the point of sustained clinical stabilization. There will be a strong focus on management of members with heart failure.

Job Responsibilities/Duties

  • Performs ongoing nursing assessments and care, focusing on physiologic, psychosocial, cultural, spiritual and cognitive status.  Identifies patient/family learning needs and implements appropriate measures.  Provides education to patients and families about disease processes, symptom management, and advance care directives.  Formulates a goal-directed plan of care that is prioritized and based on determined nursing diagnosis and patient outcomes. Perform case management duties according to Prospect’s policy and procedure, accepted standards of care and clinical guidelines.
  • Responsible for timely and clear documentation of findings, interventions in care management platform/Electronic Health Record per approved protocol.  Communicates significant information to appropriate team members. 
  • Provide feedback for program development.  Participates in departmental and interdisciplinary meetings, task forces and projects as required.  Contributes to the development & implementation of quality, service & performance improvement processes related to the delivery of nursing services.
  • Perform other related duties as required.  


Minimum Education: Graduate of an approved, accredited school of professional nursing required.

Minimum Experience: Minimum of two (2) years direct patient care experience required; cardiology, critical care, case management experience preferred. Knowledge and understanding of chronic disease management process with an ability to identify symptoms and activate action plan with members. A high degree of organizational skills, ability to set priorities, manage multiple demands and the ability to complete tasks under strict timelines is required. Ability to establish therapeutic relationships with patients and families. Ability to document in Case Management platform and/or Electronic Health Record. Proficient at Microsoft Office products (Excel, Word, Teams, etc). Effective written and verbal communication skills. Maintains professional and personal integrity. Ability to work independently as well as in a team environment. Demonstrated ability to be self-directed, and motivated. Current driver’s license, a good driving record and reliable transportation required.

Req. Certification/Licensure: Current unrestricted license to practice as a Licensed Vocational Nurse in CA required, RN preferred. Current CPR certification required. Current driver’s license, a good driving record and reliable transportation required.
