Director of Clinical Operations, Care@Home Solutions

Responsible for management and oversight of day to day clinical operations of Care@Home program. Provides & Implements operational strategies and clinical processes improvement to ensure Care@Home program’s quality, growth and financial metrics are achieved.

Job Responsibilities/Duties

• Develops and implements clinical protocols and operational SOPs including Care@Home referral criteria and enrollment, patient outreach and scheduling, in home and tele visits, best practice guidelines for complex and chronic disease care management & coordination for in-home care setting, specialty referrals, AWV, TOC, palliative care, and multidisciplinary team approach and care coordination.
• Provides analytics, develops key performance indicators, and tracks quantitative and qualitative data to ensure Care@Home program is meeting the quality, compliance and financial goals and metrics identified by senior leadership
• In collaboration with both internal and external stakeholders build sustainable and fully integrated Care@Home Solutions, and manage interdepartmental teams (HR, contracting, IT, medical management, compliance, quality performance, network and business development)  in the implementation, enhancement and optimization of Care@Home Solutions’ people, processes, and systems.
• Provides leadership, mentoring and management to Care@Home clinical (LVN, RN Case Managers, social workers, and registered dietitian, etc.) and supporting staff (clinical care coordinators, MAs, and outreach schedulers) while using tools to measure productivity & progress on metrics.
• Coordinates new staff onboarding process and conducts ongoing staff & provider trainings for EMR, practice management, and other clinical applications and clinical services to ensure quality and compliance of the program.
• Ensures consistency in communication and standardization and efficiencies of workflows. Evaluates staff performance and adjusts staff compensation packages to support changes in the marketplace and company goals. 
• Supports and leads other internal operational projects as directed including in collaboration with Care@Home CMO and clinical leadership implement and support Care@Home’s new market expansion efforts and organizational growth.

Qualifications

Minimum Education: Bachelor Science in nursing and/or health care administration preferred.

Minimum Experience: Minimal five to seven (5-7) years of clinical operations experience developing and fully operationalizing outpatient clinic programs with an emphasis on comprehensive care coordination in home care settings. Experience in staff training, clinical applications, data analytics, EMR/health, IT and clinical operations in the health care setting. Demonstrates in-depth knowledge in clinical program operations in home care setting and healthcare delivery focusing on comprehensive care model and practice management. Comprehensive knowledge and understanding of healthcare finance and operations as well as healthcare technology. Must possess the ability to simultaneously address multiple projects, demands, and issues. Demonstrate effective critical thinking and decision- making skills. Extensive knowledge with regulatory policies of NCQA, CMS, etc.  and working knowledge of health care compliance & risk management. Requires interpersonal and leadership skills and excellent written and verbal communication skills.

Req. Certification/Licensure:  Active RN License in the State of CA required.
