CRC Physical Therapist – Rhode Island

CRC is Population Health organization with a goal of maximizing the health and wellbeing at home.

The CRC Physical Therapist (PT) builds and implements high-quality Physical Therapy services with compassion in partner hospitals. The PT is responsible and accountable for triaging patients who would benefit most from getting physical therapy services in hospital and with that avoid institutionalization. The PT is responsible for performance of all physical therapy duties and modalities including the assessment, goal setting, treatment planning, therapeutic interventions, and discharge planning to help patients overcome the barriers to return to home from Hospital.  Works with patients, patients care givers, and hospital care managers to gather patient’s previous level of functioning, current level of functioning and builds a therapy plan to help patients maximize their chances of returning home safely.  Prioritizes patients’ medical, environmental, and psychosocial needs with the goal of optimizing physical status to facilitate discharge to home. Provides staff, patient and family education, and initiates and updates patient care plans.

Job Responsibilities/Duties

  • Assesses patients need for physical therapy services by validating physician orders, interviewing patients, performing assessments, treatment planning, and provide appropriate recommendations. Initiates and conducts therapeutic interventions, evaluates treatment efficacy, and modifies prescribed therapeutic interventions as needed.  Counsels and educates patients, families and care givers.  Participates in department and hospital wide performance improvement initiatives.
  • Identifies patients and provides therapy services for whom providing physical therapy/occupational therapy in hospital will make a meaningful difference in ability to return to home without requiring inpatient rehabilitation/skilled nursing facility stay.  Collects information on previous level of functioning and current level of functioning.  Triages the patients based on gap between current functional ability and the level of functioning required to return to home.
  • Creates and shares a post-discharge plan of care with SNF Care Manager, Home Health Team, and others as needed.  Proactively communicates with others in CRC to ensure CRC team can support and supervise discharge plan as well as ensure optimal patient care.
  • Evaluates all clinical data to determine the appropriateness of physical therapy services, participates in the development of the care plan, and provides care using clinical protocols.  Ensures timely, accurate and complete patient clinical documentation to include charges and billing.  Maintains institutional compliance with professional, accrediting and regulatory standards of care.  Ensures Infection Control procedures and HIPAA compliance. 


Minimum Education: PT unrestricted active license for State of RI

Minimum Experience: Four (4) years experience as a PT preferably in an acute care setting or home health care. Demonstrated ability in gaining trust. Strong clinical background to assess patients, educating families and patients. Ability to learn and develop a plan to help patients in their period of recovery (transitions of care plan). Experience with Microsoft Office Word and Electronic Health Records. Valid unrestricted Driver’s license. Willingness to provide therapy in acute care/hospitals as assigned. Ability to manage, prioritize and handle action items, care of 10-13 patients simultaneously (not all of them would need or benefit from PTA). Ability to work with patients, be respectful of patient autonomy and persuade patients and families. Ability to work independently and be comfortable with making independent decisions while soliciting input from other colleagues, as needed. Ability to multi-task. Excellent written and verbal communication skills. Maintain courteous professional attitude when working with internal and external customers. Maintains member confidence and protects operations by keeping claim information confidential in compliance with HIPAA requirements. Ability to work shifts including weekends, and holidays (negotiable).

Req. Certification/Licensure: PT unrestricted active license for State of RI. Valid unrestricted Driver’s license.
